Case Management Services Program


This project is made possible through funds under Title III-B
of the Older Americans Act of 1965, as amended,
administered by the Department of Public Health
and Social Services, Division of Senior Citizens.

Service Provider: Health Services of the Pacific

This program helps seniors who are aged 60 or older with
physical or cognitive limitation who need assistance managing
activities of daily living.

National Family Caregiver Support Program


This project is made possible through funds under Title III-E,
Older Americans Act of 1965, as amended, administered by
the Department of Public Health and Social Services,
Division of Senior Citizens.

Service Provider: Health Services of the Pacific

This program offers support to caregivers to obtain information
on available services in the community. NFCSP also provides
support groups and supplemental services.
